Tailored Care Management Services for Families

NC Tailored Care Management is a North Carolina Medicaid service for eligible people with significant behavioral health, intellectual or developmental disability, or traumatic brain injury needs. It coordinates medical, behavioral health, and community-based supports through a designated care manager. Helpr is an employer-sponsored family-care benefits platform and does not represent itself as a North Carolina Medicaid Tailored Care Management provider.

A tailored care manager generally helps an eligible individual assess needs, create a person-centered plan, coordinate providers and services, and monitor progress. In North Carolina, the role is associated with Medicaid Tailored Plans and specific eligibility rules. Helpr’s care specialists serve a different purpose: they help employees explore and arrange family-care options through an employer’s Helpr benefit.

Medicare may cover Transitional Care Management, often called TCM, when a qualified clinician provides follow-up after discharge from an inpatient setting, observation stay, or certain other facilities. This clinical service includes timely contact and a follow-up visit, subject to Medicare requirements. It is different from North Carolina Tailored Care Management and from Helpr’s employer-sponsored care benefits.

This service is especially useful when standard backup care does not address a household’s ongoing care requirements.

Yes. Helpr’s patented My Choice technology can allow employees to use eligible employer subsidies to pay people within their own trusted support network, such as family members or friends. This option recognizes that families often know which care arrangement works best for them. Specific eligibility, subsidy amounts, and payment rules depend on the employer’s Helpr program.

Helpr uses a use-based model in which organizations pay for care services that are actually used under their selected benefit design. Employers can choose to subsidize care to reduce employees’ out-of-pocket costs. For certain services, listed appointment pricing includes taxes and Helpr’s service fee; minimum appointment, overtime, and early-return rules may apply.
